이스라엘의 스타트업 Ophthalmic Sciences가 세계 최초로 아이오퍼펙트(IOPerfect)라는 AI 기반 안압(Intraocular Pressure, IOP) 측정기기를 선보였다. IOPerfect는 원격환자모니터링(remote patient monitoring, RPM) 장치로 원격의료(telehealth) 기능을 제공한다. 이 장치는 비대면으로 자기기입식 AI 기반 안압과 시각 분석을 제공한다.
Israeli startup Ophthalmic Sciences has released the world’s first AI-based intraocular pressure (IOP) measuring device called IOPerfect. IOPerfect is a remote monitoring device for the treatment of glaucoma and it enables telehealth, which is healthcare provided remotely. The device uses AI-based visual analysis of ocular vascular pressure response that is self-administered and contact-free (untact).
IOPerfect는 VR 헤드셋으로 기압과 카메라를 활용해 내·외부 혈관의 압력에 반응하는 차이를 포착한다. 안약 없이도 언제든지 2분 이내에 자가검사 가능하며, AI 알고리즘이 처리된 영상을 분석해 환자의 안압(IOP) 수준을 계산하고 그 결과가 즉시 임상의에게 전송돼 상담이 이루어진다.
IOPerfect is a VR-like headset that applies mild controlled air pressure and utilizes micro-video cameras to capture the difference in response to pressure on the internal and external blood vessels. The self-administered test can be done anytime, without eye drops, and done within two minutes. The AI algorithm will analyze the processed video to calculate the patient’s IOP level and the results will be immediately transmitted to the clinician for consultation.
Ophthalmic Sciences는 백내장의 조기 발견, 망막중심정맥폐쇄(Central Retinal Vein Occlusion), 망막지정정맥폐쇄(Branch Retinal Vein Occlusion) 등 안구 운동 장애와 관련한 의학적 응용을 위해 기술을 더욱 발전시키고 있다. IOPerfect는 VR 기반 비대면 안과 의사를 두는 것과 같이 안과와 관련된 대부분의 사항에 대해 완벽한 원격 환자 모니터링 장치로 작용할 수 있을 것으로 기대된다.
Ophthalmic Sciences is further developing the technology for other medical applications such as early detection of cataracts, central or branch vascular occlusions, and other ocular motor disorders. IOPerfect may one day be a perfect remote patient monitoring (device) for most things ocular-related, like having a VR-based, untact ophthalmologist.
녹내장에 대한 지속적인 모니터링의 필요성 (The need for constant monitoring for glaucoma)
녹내장은 안압이 높아져 발생하는 만성 안구질환으로, 방치하면 시신경이 손상된다. 전 세계적으로 녹내장을 갖고 있는 개체는 1억 5천만 마리로 추산되며, 그중 50%가 진단되지 않은 개체이다. 또한 전 세계적으로 녹내장은 실명의 두 번째 주요 원인으로 꼽힌다. 안압은 녹내장의 핵심 바이오마커이며 수개월마다 테스트를 수행해야 하므로 시간과 비용이 많이 소요될 수 있다. 다만, 속발녹내장(secondary glaucoma)인 홍채각막내피증후군(iridocorneal endothelial syndrome, ICE syndrome)과 같은 다른 형태의 녹내장도 존재하기 때문에, 이러한 VR 헤드셋의 카메라와 인공지능이 다른 안과나 일반 안과 검사로 확장될 수 있기를 기대한다.
Glaucoma is a chronic ocular disease caused by elevated IOP that damages the optic nerve if left untreated. It is estimated that there are 150 million individuals in the world with glaucoma, and 50% of them are undiagnosed. Glaucoma is also the second leading cause of blindness globally. Intraocular pressure (IOP) is the key biomarker for glaucoma and require testing every few months, which can be time and cost extensive. However, there are other forms of glaucoma such as iridocorneal endothelial syndrome (ICE syndrome), which is a secondary glaucoma. And hopefully the cameras and AI in the VR headset can be expanded to other ocular conditions or even a general ophthalmology exam.
